Yeah, I would have that look if a doctor made a weird alien joke about where things are placed...but this isn't NEW to Star Trek.

The first instance of a "sexual organs on the knee" joke was in the Rura Penthe mines, when Kirk bashes the horned or knee-jerk alien. Just and unnamed character with a kick to the nards.
